Functional outcomes – Educational success; Employment; Absence of Court Involvement at Follow-up; Absence of Abuse at Follow-up. |

Functional outcomes relate to the child’s ability to live successfully in the community.

Effectiveness of Placement – Restrictiveness of Placement at Discharge and at Follow-up (more, less, same, runaway), Nature of Discharge (planned, runaway, removed, administrative), and Meeting the Child’s Permanency Plan. |

Placement outcomes relate to where the child is placed at discharge. |

Consumer satisfaction – Parent, child, referring agency satisfaction |
In addition to the key outcomes identified above, the following information is also obtained: |

Risk Factors – Demographic variables, child and family risk factors, total risk factor score. |

Services – Key services provided during placement in the areas of Substance Abuse, Early Childhood, Educational, Employment, Family-Based, Legal, Medical, Mental Health, Recreational, and Rehabilitation. |
